Reports to: Lead Practice Nurse/Lead GP

Location: We are currently looking for Practice Nurses to be primarily based in the locations below:

• Penrose Surgery, 33 Penrose Street Walworth, Southwark, London SE17 3DW
• Kingfisher Surgery, Staunton Street Lewisham, London SE8 5DA

Salary Band: £38,000 – £45,000 per annum (depending on experience)

Advertised Band: £38,000 – £45,000 per annum (depending on experience)

Hours: Full-time or part-time

Purpose of the role

To deliver safe, evidence-based nursing care within a multidisciplinary primary care team, supporting patients across South East London. The Practice Nurse plays a key role in managing long-term conditions, promoting preventative care, and contributing to clinical quality frameworks such as QOF and local enhanced services.

Key relationships

• Clinical Lead (in-surgery) – for day-to-day guidance and clinical oversight.
• Nursing Lead (cross-group) – for professional development, peer support, and shared best practice.
• Multidisciplinary Team – including GPs, Pharmacists, HCAs, Nursing Associates, and administrative staff.
• Patients – providing direct care, education, and support across diverse communities.

Key responsibilities

• Manage long-term conditions (e.g. diabetes, asthma, COPD) through routine reviews and proactive care.
